Along with the Clearances, it is necessary to complete the two (2) forms attached here:
1) Volunteer Registration Form
2) Confidentiality/FBI Exemption Form
In addition, to be an approved school volunteer, a TB test dated within the past three (3) months must also be submitted with your packet.
